About Our Office

Our office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. Patients wishing to make an appointment may call 796-0202, or dial toll free 800-658-6262. We accept Medicare assignment. We are on the preferred provider list of many insurance plans. If your plan does not list this office as a provider, please let us know and we will consider applying for PPO status.
